  • 本人同意遵守一切選舉規則及願意成為「香港創新領軍人物大獎」之被提名人。
  • 本人謹此聲明，就本人所知，上述資料真確無訛。如獲選為「香港創新領軍人物大獎」得主，本人允許主辦機構公開此等資料及附件。
  • 本人已細閱及同意遵守選舉規則，並服從評審結果。
  • 本人同意及確認「香港創新領軍人物大獎」之主辦機構、甄選委員會及評審委員會對本人之資料作審核及向提名人和有關機構查取進一步資料。
  • 如獲選為「香港創新領軍人物大獎」得主，本人同意將結果保密直至頒獎典禮晚宴之正式公布。本人將出席於2019年7月3日晚上舉行之頒獎典禮晚宴，並同意協助主辦機構進行有關之宣傳活動。
  • 本人同意此表格上的所有資料以及所有提交之文件將向主辦機構、甄選委員會及評審委員會披露。
  • I agree to comply with the rules and regulations of “The InnoStars Award” and am willing to be nominated for selection.
  • I declare that to the best of my knowledge, all the information supplied in this nomination form is true and correct. I also grant permission to publish these information to the organiser should I become a winner of “The InnoStars Award”.
  • I have read and agreed to the rules and regulations of entry, and I shall accept the judging results.
  • I agree and acknowledge that the Organiser, Vetting Committee and Judging Panel may verify the information and documents submitted herewith and obtain further reference from the nominator and organisations concerned.
  • If I am selected as a winner of “The InnoStars Award”, I agree to keep the results confidential until the official announcement at Award Presentation Ceremony. I shall attend the Award Presentation Ceremony to be held on 3 July 2019 and assist the Organiser in associated publicity activities.
  • I consent to disclose all the information provided in the form and all documents and materials submitted to the Organiser, Vetting Committee, and Judging Panel of the Award.
